Paraprofessional

Description:

Provide direct services and works independently in clients’ home and other community settings, utilizing creative clinical interventions based on best practice models and learned counseling techniques to work effectively with children, adolescents, adults, and families.

Primary Responsibilities (include but are not limited to):
  • Develop treatment plan with client and family participation.
  • Facilitating and managing the counseling process according to the counselor’s guiding theory.
  • Demonstrating therapeutic communication skills consistent with counselor’s theoretical framework.
  • Attend and actively participate in supervision meetings. Be prepared to discuss current caseload.
  • Attend and actively participate in all treatment team and other staff meetings.
  • Attend and actively participate in all required trainings including ELearning.
  • Document and report service delivery and client progress on an ongoing basis on progress notes, treatment plan reviews and thorough discharge summaries.
  • Abide by laws, regulations and standards of practice regarding Confidentiality and Ethics for the provision of treatment under the scope of license and certification.
  • Perform other duties as assigned.
Qualifications:
  • Master’s degree or a Bachelor’s degree in a Mental Health, Counseling or Human Services discipline.
  • Possess a valid Driver’s License;
  • Have a clean Motor Vehicle Records (MVR).
  • Have a clean Criminal Back Ground Check (no convictions or shown credible evidence of perpetrating abuse, neglect or sexual exploitation – child or adult.

Hybrid-Therapist

Description:

Provide counseling, therapy, and/or psychotherapy to clients and families as appropriate to needs.

Primary Responsibilities (include but are not limited to):
  • Provide assessments, individual and group counseling, family counseling, and other treatment modalities within an evidence-based framework.
  • Strong clinical judgment in assessment, application of diagnostic criteria, clinical formulations, and referral to appropriate level of care.
  • Experience providing clinical services to substance using clients.
  • Experience working with Adults and Veterans.
  • Experience conducting substance use groups and competent in providing services to co-occurring population.
  • Prepare treatment plans, discharge summaries, monthly summaries, BIRP Note and crisis intervention, as well as emergency services as needed.
  • Attend and actively participate in all treatment team and other staff meetings.
  • Abide by laws, regulations and standards of practice regarding Confidentiality and Ethics for the provision of treatment under the scope of license and certification.
  • Perform other duties as assigned.
Qualifications:
  • Master’s degree in a Mental Health, Counseling or Human Services discipline; LCMHC, LCSW, LMFT, or LPC in GA.

Substance Abuse Counselor

Description:

Provide substance abuse education, skill training and treatment to consumers with substance abuse disorders and co-occurring disorders.

Primary Responsibilities (include but are not limited to):
  • Conduct and document initial and on-going assessment of client’s strengths, bio psychosocial histories, substance abuse and mental health treatment needs.
  • Under supervision and with consultation of Clinical Director, provide crisis assessment and intervention.
  • Ensure appropriate safety plan is established.
  • Through the use of acceptable therapeutic interventions and techniques, assists clients in establishing and reaching treatment goals.
  • Document and report service delivery and client progress on an ongoing basis on progress notes, treatment plan reviews and thorough discharge summaries.
  • Responsible for individual, family and group therapy sessions.
  • Attend and actively participate in supervision meetings. Be prepared to discuss current caseload.
  • Attend and actively participate in all treatment team and other staff meetings.

Qualifications:
  • Certified as a Certified Addiction Counselor I or II by GACA a plus.
  • Have an understanding of and sensitivity to serving the substance and mental health population.
  • Possess a minimum of a High School diploma or equivalent.
  • Possess a valid Driver’s License.

Registered Nurse / LPN

Description:

Provide direct services and works independently in clients’ home and other community settings to monitor, evaluate, assess, and/or carry out nursing assessments and the physician’s orders regarding the psychological and services requirements relating to physical problems and general wellness. To work effectively with children, adolescents, adults, and families.

Primary Responsibilities (include but are not limited to):
  • Provide nursing assessments and interventions to observe, monitor and care for physical, nutritional, behavioral health and related psychosocial issues, problems or crises manifested in the course of the client’s treatment.
  • Consult with client’s family members about medical, nutritional and other health issues related to the individual’s mental health or substance related issues.
  • Educate the client and family members on medications and potential medication side effects.
  • Obtain medical history, particularly as it relates to the present condition, from the client and / or family member(s)
  • Prepare clinical and medical progress notes and summaries of care.
  • Ensure proper maintenance of medical records.
  • Set up doctor’s appointments for clients to be evaluated by psychiatrist.
  • Abide by laws, regulations and standards of practice regarding Confidentiality and Ethics for the provision of treatment under the scope of license and certification.
  • Perform other duties as assigned.
Qualifications:
  • Minimum Associates Degree in healthcare field.
  • Current Georgia Licensure at an RN / LPN level.
  • Certification as CAS, CACII, CCS, and/or CADAC II level is preferred to provide counseling.
  • First Aid/CPR Certified.
  • Excellent organizational and critical thinking skills.
  • Ability to work independently in home or alternate site settings.
  • May need to assume a flexible work schedule.
  • Possess a valid Driver’s License.
  • Have a clean Motor Vehicle Record (MVR).
  • Have a clean Criminal Back Ground Check (no convictions or shown credible evidence of perpetrating abuse, neglect or sexual exploitation – child or adult.

Clinical Assessor

Description:

Perform client assessments and re-assessments for clients in community mental health and substance programs as needed.

Primary Responsibilities (include but are not limited to):
  • Responsible for the assessment and re-assessment of referred clients and current clients.
  • Responsible for the writing up and input of assessment data.
  • Responsible for the input of authorization data as necessary.
  • Consult with Clinical Director as needed with regards to any client concerns that arise during the assessment process.
  • Provide individual/group/and or family therapy as needed and responsible for corresponding service documentation.
  • Provide crisis intervention as needed.
  • Responsible for the maintenance of all applicable State, Federal, and Local standards with regards to performance of the assessment and re-assessment process.
  • Conduct verbal behavior and functional assessments, developing treatment plans, directly implementing the treatment plan, analyzing progress data, and modifying the treatment plan as necessary (DSM diagnosis and biopsychosocial assessments).
  • Solid clinical writing skills.
  • Performs other duties as assigned.
Qualifications:
  • Licensure required; LCSW, LPC or Licensed Psychologist.
  • Master’s Degree in Behavioral Science or Clinical field.
  • 1-year experience in a crisis setting with individuals and families.
  • Bilingual (English/Spanish) a plus.
  • First Aid/CPR Certified.
  • Ability to work independently in home or alternate site settings.
